What is MyChart?

MyChart offers you secure online access to portions of your medical record. You may also know it as MyHealth.

MyChart makes it easy to: E-mail your doctor’s office Get test results Schedule appointments View visit notes Participate in virtual video visits with your provider

Becoming a MyChart user is easy!

• Go to http://www.essentiahealth.org/MyChart. Click the Sign Up Online button to sign up without an activation code. Complete an electronic authorization and you will be able to access MyChart right away.  
• Or sign up in your doctor's office.

MyChart should never be used for urgent medical issues. For medical emergencies, dial 911.

Once you have a MyChart account get the app to access your account and quickly see new messages. Simply put your phone camera over the QR code below to download the MyChart app.  Already have a MyHealth account?  Download the MyChart app and use your same information to log-in.
